Please note that approximately twenty (20) minutes are required to complete the official Ecclesiastical Baptism Application. 

All of the following documents must be submitted prior to the initiation of the application: 

1. The Catechumen’s/Child’s Certificate of Live Birth or valid identification
2. The Catechumen’s Baptismal Certificate (if previously baptized, where applicable) 
3. The Godparent’s (Sponsor’s) Baptismal Certificate (Orthodox)
4. The Godparent’s (Sponsor’s) Letter of Good Standing

Please Note: 

• The Sponsor must be an Orthodox Christian in good standing.
• All documents must be original or certified copies.
• Catechetical preparation may be required.
• No baptism date may be scheduled until all requirements are fulfilled.
